Miami-Dade County residents only, 18 years or older.
To qualify for discounted services, please bring documentation of one of the following to your appointment: Public Housing, Supplemental Security Income, Disability/SSD, Medicaid, Welfare, Temporary Assistance for Needy Families (TANF), Food Stamps, WIC, Food and Nutrition Services, Jackson Memorial Hospital Clinic Care.
